Prolotherapy Q & A

What is prolotherapy?

Prolotherapy — short for proliferative therapy — is a medical technique that uses injectable solutions, typically made with dextrose or sugar, to irritate tissues in order to trigger an anti-inflammatory immune response. The technique has been around since the 1930s and has become popular in recent years as patients want more nonsurgical and minimally invasive solutions to chronic joint, tendon, and muscle pain.

What conditions can benefit from prolotherapy?

Prolotherapy is most often used for chronic soft tissue and joint injuries that have failed to heal properly. Common conditions that benefit from prolotherapy include:

  • Tennis Elbow
  • Golfer’s elbow
  • Jumper’s knee
  • Achilles tendinitis
  • Hip and shoulder bursitis
  • Cartilage, ligament, and joint damage
  • Plantar fasciitis

Different from cortisone injections that only reduce inflammation, prolotherapy goes further to initiate a healing response in injured tissues which provides long-lasting benefits and more successful treatment outcomes.

How does prolotherapy work?

There are different ingredients that can be used for prolotherapy injections, depending on your needs. These ingredients contain an inflammatory substance — typically dextrose or sugar — along with growth factors to help repair and rebuild damaged tissues. When your injection is performed, the specific ingredients first reduce inflammation in the injured tissues. With less inflammation, there’s less pain and swelling and a greater range of motion and function in the affected area.

The additional growth factors supply essential components to repair injured tissues — including muscles, tendons, ligaments, and cartilage. Over a period of weeks or months, your body naturally repairs these tissues to improve the function and feel of the area long-term.

What should I expect when getting prolotherapy?

Before your treatment begins, the team performs a thorough consultation and exam to determine what type of prolotherapy solution is best for your condition. Once that’s determined, you receive an injection, guided by ultrasound imaging, into the injured tissues. This injection takes less than 10 minutes and may be repeated three or four times over the next four to six weeks before being reevaluated.

Depending on how you’re healing after your initial series of visits, the team may recommend additional treatment, either with the same solution or a modified prolotherapy solution. In many cases, you may notice a decrease in the levels of pain and inflammation in the injured area within two to four weeks, at the earliest.
